Peroneal Tendon Injury Q & A

What is peroneal tendonitis?

  • The peroneus longus and peroneus brevis tendons run in a groove along a bony prominence on the fibula bone (on the outside of the ankle). Peroneal tendonitis is pain, inflammation, or irritation of the tendons along the outside of the foot and ankle.

What can cause peroneal tendonitis?

  • Sudden injuries like rolling the ankle
  • Previous or recurrent ankle sprains
  • Repetitive stress or overuse of the peroneal tendons – running long distances, sudden increase in training intensity, jumping, etc.
  • High arch foot type

What are the symptoms of peroneal tendinitis?

  • Swelling, redness, or increased warmth around the tendon
  • An increase in the thickness of the tendon
  • Snapping feeling along the tendon
  • Ankle weakness or instability
  • Pain that can become worse with physical activity
  • Peroneal tendinitis can become chronic if left untreated and can lead to a tendon tear

How is peroneal tendinitis treated?

  • Rest, immobilization, over-the-counter NSAIDs (non-steroidal anti-inflammatories such as ibuprofen or naproxen), activity modifications, ankle bracing, supportive shoes, orthotics, physical therapy, shockwave therapy and injections. In rare cases if the tendon doesn’t respond to basic conservative treatments, then surgery is performed.
